Chronic illness and mental health item numbers

When a patient has a mental health illness as well as significant comorbidities and complex needs requiring team-based care, you can use both chronic disease management (CDM) item numbers and a GP MHTP (MBS items 2715, 2717).

Although you’re not precluded from managing a patient under both the CDM item numbers and a GP MHTP, you should consider whether developing two separate care plans is necessary. As a general principle, you should avoid creating multiple plans unless the patient clearly needs an additional care plan to manage a separate medical condition.
